When you attend the appointment you should have the following required documents:

(a) $200 registration fee (T1-K4) or $525 registration fees (K5-12)

(b) your student’s birth certificate,

(c) Social Security Card or F1 Visa,

(d) 680 Certificate of Immunization,

(e) 3040 Certificate of Physical Examination,

(f) Official academic transcripts.
